Bert Slagmolen used Weick and Sutcliffe’s model of mindfulness in organizations similar to Libuser, that is, error does not lead to death and may not cause catastrophe.  In these organizations, failure, while catastrophic, occurs with poor service rendered to the customer or public.  In this context, organizations do not seek reliability but performance and become High Performance Organizations (HPOs).
